Our People
At Tranquil Pathway Homes, we prioritize person-centered care. Our trained staff ensures clear communication with families and case managers, embodying C.A.R.E.S.: Compassion, Accountability, Respect, Empowerment, and Sensibility.
Groups Served
We provide person-centered support for three primary groups:

Individualized support with mobility, personal care, and daily living activities

Encouragement of autonomy so residents feel supported to do as much as possible, independently, if comfortable enough

Promotion of independence through skill development and meaningful engagement

Implementation of structured support with daily living activities, communication, and community participation

Emphasis of respect in regard to each resident’s unique abilities, preferences, and goals

Support through holistic care with medication management, mobility assistance, and coordination of medical needs

Promote aging in place with stability, meaningful connection, and an enhanced quality of life.

Five welcoming homes designed around comfort, accessibility, and individual routines.
Home 1
Four-resident, handicap-accessible home licensed as a 3–4 bed non-ambulatory adult family home.
Downstairs
A cozy setting for one to two residents.
Emery
A welcoming two-bed home with a backyard garden.
